Sunvozertinib Moves Into the First-Line Setting for EGFR Exon 20 NSCLC – Oncogene Cancer Research

“For people with EGFR exon 20 lung cancer, chemotherapy has been the standard first-line option. These mutations are notoriously difficult to target with conventional EGFR inhibitors, leaving limited precision choices early on.

But what if we could offer a targeted therapy from the start, rather than saving it for later treatment lines?

The WU-KONG28 trial, a phase 3 international study published in The New England Journal of Medicine (NEJM Group), tested exactly that.

Researchers compared sunvozertinib to standard chemotherapy (carboplatin-pemetrexed) as first-line treatment for advanced NSCLC with EGFR exon 20 insertions. The targeted approach came out ahead:

  • Progression-free survival: 10.3 months vs 7.5 months with chemotherapy (35% lower risk of disease progression or death).
  • At 12 months, 46.1% on sunvozertinib remained progression-free, vs 26.7% on chemo.
  • Response rate: 58.9% vs 31.1% in favour of sunvozertinib.

From a patient perspective, this matters. People with EGFR exon 20 mutations may now have a precision medicine option as a starting point. The stronger side effect profile is real, but the survival and response data suggest it’s a worthwhile trade. Overall survival data were still maturing, so longer-term follow-up will be important – but these results point toward a genuine advance for this specific mutation.

This is precision oncology working: match the right drug to the right mutation, from the beginning, and improve outcomes.”
