Henry Fung, Co-Chair of Department of Cancer Medicine at Fox Chase Cancer Center, shared on X:
“Elranatamab is moving from salvage to center stage in multiple myeloma.
- MagnetisMM-3 established activity in advanced, triple-class-refractory disease.
- MagnetisMM-5 moved into earlier relapse—largely anti-CD38-naïve or daratumumab-sensitive.
- MagnetisMM-32 extended the strategy to anti-CD38- and lenalidomide-exposed disease, with a positive PFS topline.
Now come the two most consequential challenges:
- MagnetisMM-6: EDR versus D-VRd – challenging the frontline crown.
- MagnetisMM-7: elranatamab versus lenalidomide after ASCT – challenging maintenance.
But MM-7 raises an important question: is lenalidomide alone still the right comparator? The more contemporary challenger should arguably be Dara-R – and perhaps Isa-R.
Elranatamab is becoming a major player in myeloma. The remaining questions are not only whether it wins, but where it belongs, how long to give it, and whether improved PFS ultimately translates into better OS and treatment sequencing.

What’s NEXT?
Elranatamab has already moved from advanced RRMM toward earlier relapse, frontline therapy and maintenance.
Now comes the harder question: can it become a better platform?
- MagnetisMM-5: Does adding daratumumab deepen and prolong response enough to justify additional immune suppression?
- MagnetisMM-30: Can iberdomide amplify T-cell function?
- MagnetisMM-20: Can orthogonal killing – with Kd or CD47–SIRPα macrophage activation – overcome resistance?
- MagnetisMM-9 and phase III schedules: Can we give less frequently, reduce infection and treatment burden, yet preserve disease control?
MagnetisMM-20 is particularly intriguing – but it is an all-comer RRMM dose/safety study, not a PET-defined EMD trial. It will not answer whether combined T-cell and macrophage engagement can overcome the extramedullary niche.
The next frontier is no longer simply where to use elranatamab – but how to make it deeper, safer and more durable, with less exposure.”
