A randomized trial in NEJM AI (June 2025) tested the AI-Standardized Clinical Examination (ASCE) on the DocSimulator platform (GPT-4). It compared AI-based training with traditional OSCE preparation for early-clerkship students.

Evaluating whether AI-driven simulations can improve medical students’ clinical performance, readiness, and stress management compared to standard OSCE prep.
Key Findings
- Higher OSCE scores with ASCE (median 11.4 vs. 10.7; P=0.02)
- Absenteeism reduced (0.8% vs. 4.9%)
- Improved emotional readiness (P<0.001)
- Lower stress (P=0.02)
- High realism: 74% felt they were interacting with real patients; 67% believed feedback came from live teachers
Conclusion
ASCE training improved competence, reduced stress, and provided realistic simulations — showing promise to reshape medical education and reduce reliance on traditional OSCEs.
