“GROUNDBREAKING STUDY: Prophylactic Surgeries Improve Survival in Young BRCA Carriers.
For the first time, a landmark international study published in The Lancet Oncology provides compelling evidence that risk-reducing mastectomy (RRM) significantly improves survival outcomes in BRCA carriers under 40 with previous breast cancer
Key Primary Findings:
• Global cohort of 5,290 young BRCA carriers across 109 centers.
• RRM associated with 35% reduction in mortality risk.
• Benefits observed regardless of specific BRCA gene, tumor subtype, or age.
Additional Objectives Achieved:
• Risk-reducing salpingo-oophorectomy (RRSO) also significantly improved overall survival (42% reduction in mortality).
• RRSO showed particularly strong benefit in BRCA1 carriers (56% reduction).
• Both procedures reduced incidence of second primary cancers by more than half.
• Significant improvements in disease-free survival and breast cancer-free interval.
• Identified benefits regardless of timing of BRCA testing (before or after diagnosis).
• Provided evidence for optimal surgical approaches in different patient subgroups.

Until now, evidence was lacking for this unique high-risk population. These findings provide critical guidance for oncologists and surgeons counseling young BRCA carriers on cancer risk management strategies, particularly balancing primary cancer relapse risk against second malignancy risk.
