Salvador Jaime-Casas: Predicting Occult Lymph Node Involvement in Bladder Cancer
Salvador Jaime-Casas, Postdoctoral Research Fellow at City of Hope, shared a post on X about his recent article, coauthored with colleagues, published in Clinical Genitourinary Cancer:
“JUST PUBLISHED at CGC journal:
Evaluating Clinical and Pathological Predictors for Occult Lymph Node Involvement in Patients With Clinical Node-Negative Bladder Cancer Undergoing Radical Cystectomy
- Occult LN involvement occurs in 25% of patients with cN0 MIBC.
- LN involvement can significantly affect oncologic outcomes, with 5-yr OS rate of approximately 32%, compared to 75%in patients without LN involvement.
- We leveraged our institutional database to determine clinical and pathological variables associated with pLN+ in patients with clinically node-negative BC (cN0).
We included patients with cT1-4N0M0 BC undergoing RC-PLND
- 440 patients were evaluated.
- 81% (n=359) were pN0
- 17% (n=74) were pN+
Key variables:
LVI on TURBT, positive surgical margins, and ≥ cT2 disease showed higher odds of pN+ status. Preop hydronephrosis predicted worse RFS and OS.
Outcomes:
Patients with pN+ status were more likely to suffer recurrence and death compared to pN0 patients.
Key Takeaways:
- Positive surgical margins, LVI, and ≥ cT2 disease strongly predict pN+ findings in patients with cT1-4N0M0 BC.
- Essential to identify pts at higher risk of LN involvement to improve outcomes.
- Critical need for meticulous preop assessment and surgical precision techniques to optimize patient outcomes.
Incredibly grateful for the support of stellar mentors Abhishek Tripathi, Sumanta K. Pal, Wesley Yip, Clayton Lau, and Genitourinary Cancer Research Team, City of Hope.”
