“No ‘The Journey of Chest Wall Perforator Flaps (CWPF): 2017–2026′
From a relatively underutilized oncoplastic technique to becoming an increasingly accepted option for breast conservation surgery, the journey of Chest Wall Perforator Flaps (CWPF) over the last decade has been remarkable.
Since 2017, I have worked extensively to advance the use of CWPFs in breast reconstruction, particularly for women with small-to-medium sized breasts where volume replacement remains challenging.
What started as an effort to avoid mastectomy and latissimus dorsi flap morbidity gradually evolved into one of the largest single-center CWPF experiences globally.
Over the years, the work expanded from Lateral LICAP flap reconstruction using modified LAZY S incision:
- To advanced use of LTAP, AICAP, combined LICAP+LTAP and perforator-based techniques
- Covering defects across multiple breast quadrants
- Improving cosmetic outcomes while maintaining oncological safety
2020: Early prospective experience published, demonstrating high patient satisfaction and excellent cosmetic outcomes with CWPF reconstruction. Link
2024: Publication of one of the largest institutional CWPF series involving 150 patients, showing excellent survival, symmetry, low morbidity, and strong patient-reported outcomes. Link
2026: an international systematic review including 2591 patients across 52 studies, consolidating global evidence supporting CWPF as a promising reconstructive option in modern breast surgery. (see the link of the article below)
