“SKYSCRAPER-07: a win for atezolizumab, but not for TIGIT in unresectable ESCC
After definitive CRT, 760 patients were randomized to:
Atezo plus tiragolumab vs atezo vs placebo.
Atezolizumab vs placebo
- • PFS: 29.1 vs 16.6 mo | HR 0.74
- • OS: NE vs 36.4 mo | HR 0.69 But adding tiragolumab did NOT improve the primary PFS endpoint: 20.8 vs 16.6 mo | HR 0.82; P=0.095 OS HR 0.91.
Important caveat: because the hierarchical testing sequence failed with atezo + tiragolumab, the atezolizumab-alone OS comparison could not undergo formal confirmatory testing.
Verdict:
- Atezolizumab consolidation after dCRT looks genuinely promising.
- Tiragolumab added no benefit and increased toxicity. Another blow to the TIGIT story?
