“As an oncologist, I get excited about every new treatment option in cancer – especially when it opens a new door for patients with rare diseases and actionable genomic alterations.
But with every new approval or guideline update, I have learned to pause and look beyond the headline.
The latest ASCO Living Guideline Update (Version 2026.3.2) formally adds repotrectinib (Augtyro) as a recommended treatment option for patients with Stage IV NTRK fusion–positive NSCLC.
This is certainly encouraging news.
At the same time, the ASCO Expert Panel highlights an important point:
Outcome data for repotrectinib in the NTRK-positive population remain less well characterized, with a less mature evidence base compared with some established options.
For me, this is an important reminder:
New treatment ≠ equally mature evidence.
The development of new targeted therapies is essential, particularly for patients with rare actionable mutations. But as oncologists, our responsibility goes beyond identifying what is new.
We also need to ask:
- How strong is the evidence?
- How mature are the data?
- Which patient population was actually studied?
- How does this option compare with existing standards?
Innovation gives our patients more choices. Critical appraisal of evidence helps us choose wisely.”
