“The biggest CAR-T question in r/r myeloma is how does Anito-cel compare with Cilta-cel (CARVYKTI)?
Here is my controversial take comparing the PFS curves in the two trials featuring roughly similar patient characteristics (Cartitude 1 vs phase1 Anito-cel).
Methodology: AI to digitize published curves.
Not ideal, but unless we have an RCT, or Pharma contributes patient-level data for a propensity-matched analysis, we are not going to be any wiser.

The Anito-cel publication describes fewer toxicity events compared to Cilta-cel (but aided by the wisdom of aggressive pre CAR-T cytoreduction and preemptive management of toxicities).
The efficacy in terms of PFS appears to favor CARVYKTI. Note the plateauing for Cilta-cel corresponding to long term remission durability.
Anito-cel will be evaluated by the FDA on the iMMagine-1 pivotal Phase 2 registrational trial with 117 treated patients. It is possible that the PFS on this trial might improve from the phase 1 data.”
