“A new way to target BRCA-mutated breast cancer?
The FDA has granted Fast Track designation to ETX-19477, an oral PARG inhibitor, for BRCA-mutated HR+/HER2− unresectable or metastatic breast cancer.
Why PARG?
Unlike PARP inhibition, PARG inhibition prevents removal of PAR chains during the DNA-damage response leading to accumulation of DNA breaks, replication stress, and ultimately cancer cell death.
Early ERADIC8 phase 1 data are intriguing:
- ORR: 57% in phase 2-eligible evaluable patients
- ORR: 100% in the breast cancer cohort – although numbers remain very small
- DCR: 100%
- Grade ≥3 neutropenia 10% and anemia 7%
The activity is exciting, but this remains very early-phase data and Fast Track designation does not equal FDA approval.
Another novel DNA-damage response strategy worth watching – particularly as we think about what may come beyond PARP inhibition in BRCA-mutated breast cancer.”
