“‘Clinical Mondays’: Teclistamab monotherapy moves earlier in RRMM.
MajesTEC-9 is the first phase 3 study evaluating teclistamab monotherapy versus investigator’s choice of PVd or Kd in patients with relapsed or refractory multiple myeloma after 1–3 prior lines of therapy, including anti-CD38 and lenalidomide exposure.
Key Insights:
- 593 patients with RRMM were randomized to teclistamab or investigator’s choice of PVd/Kd
- Teclistamab significantly improved PFS versus PVd/Kd: HR 0.29, with mPFS not reached versus 8.2 months
- 18-month PFS rate was 69.8% with teclistamab versus 26.9% with PVd/Kd
- Overall survival was also significantly improved: HR 0.60
- ≥CR rate was markedly higher with teclistamab: 65.9% versus 16.8%
- Treatment discontinuation due to adverse events was lower with teclistamab despite longer treatment duration
Conclusion: MajesTEC-9 shows that teclistamab monotherapy can significantly improve PFS and OS versus standard-of-care regimens in earlier-line RRMM, supporting BCMA×CD3 bispecific therapy as an increasingly important treatment option as early as second line.
