A patient and her family drive nearly 90 minutes to my clinic for a visit lasting less than an hour. We discuss perioperative immunotherapy—and how to make treatment work without missed paychecks or added childcare. In rural America, access to modern cancer care is shaped as much by distance and logistics as by innovation.

As therapies become more effective and complex, who delivers care-and where-matters as much as the science.

Our JCO Oncology Practice workforce analysis shows a widening shortage of hematology and medical oncologists, with rural communities hit hardest. If we want breakthroughs to reach all patients, we must invest now in the community oncology workforce.
