“New collaborative European guideline for stage I–III small cell lung cancer
Delighted to share our European Society for Radiotherapy and Oncology (ESTRO)–EORTC – European Organisation for Research and Treatment of Cancer expert guideline on target delineation and radiotherapy details for stage I–III small cell lung cancer, now published in Radiotherapy and Oncology.
This collaborative effort brings together multidisciplinary European expertise to harmonise radiotherapy practice and support high-quality, consistent care across centres.
Key highlights:
- PET-CT and brain MRI are essential for accurate staging
- Concurrent chemoradiotherapy remains the standard of care for curative-intent treatment
- Recommended thoracic RT schedules include 45 Gy BID or 60–66 Gy daily using modern techniques
- Consolidation durvalumab improves survival after chemoradiotherapy
- SBRT is an option for selected patients with inoperable, node-negative early-stage disease
The guideline also addresses:
- Target volume delineation and involved-field approaches
- Motion management and image-guided radiotherapy
- Integration with chemotherapy and immunotherapy
- Role of PCI and emerging hippocampal-sparing strategies
- Future directions including adaptive RT, dose escalation and biomarker-guided approaches
These updated recommendations provide a comprehensive framework for standardised, evidence-based care and aim to harmonise radiotherapy approaches across Europe, while supporting the design of future clinical trials in limited-stage SCLC.
