Antonio Calles: New Combination Therapy Prolongs PFS in Advanced NSCLC

Antonio Calles, Medical Oncologist at Hospital General Universitario Gregorio Marañón and faculty member of the Lung and Other Thoracic Tumours faculty group of the ESMO, shared The Lancet’s post on X, adding:

“Where is the ethical justification for offering pembrolizumab alone to patients with low PD-L1 expression in this trial?

What would these patients receive if they were not participating in the study?

Chemotherapy alone?

Thats why patients participate?

To take the chance to be assigned to the experimental arm? Unfortunately a randomized phase 3 trial with no clinical implications due to a flawed trial design.”

Quoting The Lancet’s post:

“Presented at ASCO26 – OptiTROP-Lung05 phase 3 interim analysis. The findings suggest first-line treatment with sacituzumab tirumotecan (sac-TMT) plus pembrolizumab significantly prolonged PFS vs pembrolizumab alone in patients with PD-L1-positive advanced NSCLC without targetable genomic alterations.”
