“We have long known that patients with cystic pancreatic NETs fare better after resection and have lower risk of recurrence.
Also, cystic pNETs are less likely to have high-risk features such as nodal metastases. What has been missing is a really large study…
We have that now. Multi-institutional study of 1727 patients, including 177 cystic pNETs with comparison across groups. If you take care of patients with pNETs, this is a must read!
Key findings include:
- Cystic PanNETs demonstrated significantly fewer high risk histopathologic features
- Lymph node (LN) metastases (5.5% vs 24.0%, P < 0.001)
- Distant recurrence (4.1% vs 14.4%; P <0.001)
- Among tumors, ≤3 cm, cystic PanNETs had a low rate of LN metastases (3.9% vs 17.8%; P < 0.001)
- Recurrence (3.1% vs 8.4%; P = 0.041), and low propensity to recur distantly
- Cystic PanNETs had favorable long-term survival regardless of tumor size.”
