“New paper on the management of HR+/HER2- early breast cancer in the genomic era!

Here are the real-world outcomes on how intermediate-risk (i-ROR) PAM50 patients fit into the new landscape of adjuvant CDK4/6 inhibitors.

The Genomic ‘Grey Zone’

While treatment decisions are relatively straightforward for high and low Risk of Recurrence (ROR) extremes, deciding on adjuvant strategies for the i-ROR group remains challenging.

Favorable Prognosis, But Relapses Occur

A real-world cohort of 212 i-ROR patients demonstrated an excellent 5-year RFS of >96%. However, relapses still happen, predominantly in patients who are Luminal A (72.7%), node-negative (63.6%), and Cht-naive (90.9%).

Adjuvant CDK4/6i Eligibility

When applying recent trial criteria to this i-ROR cohort, 44.3% of patients were eligible for NATALEE (ribociclib) and 14.6% were eligible for monarchE (abemaciclib).

In total, nearly half (48.6%) of these intermediate-risk patients met the criteria for at least one adjuvant iCDK4/6. This highlights the need to refine risk stratification and carefully consider adjuvant systemic therapies, like iCDK4/6, even when genomic scores are intermediate.
