Spatial multi-omics of CRC macro- and micrometastases from 19 patients shows liver micrometastases emerge early as a distinct dormant, stem-like state.

They persist after therapy and are surrounded by exhausted immune cells with PD-1/PD-L1 checkpoint signaling.

A 6-gene signature (MicroMetSig) from spatial profiling predicts shorter disease-free survival, higher recurrence risk, and chemotherapy resistance.

Tissue-based markers could complement ctDNA for MRD detection.
