“Seven years after the 10-year results were first presented, NSABP B-42 is back with its FINAL analysis.
We already know the overall benefit of extending an AI toward 10 years is modest – with benefit driven in part by prevention of contralateral/second primary breast cancers and a delayed reduction in distant recurrence.
So for me, the most interesting question at #ESMO26 isn’t simply 5 vs 10 years.
It’s WHO actually benefits from years 6–10?
And will the final analysis help connect the dots with what we’re learning from genomic tools?
- Mammaprint: Will the final results align with prior B-42 translational data suggesting the greatest extended-AI benefit in low-risk, but not ultralow-risk disease?
- Breast Cancer Index: Will longer follow-up strengthen the signal that BCI (H/I)-High identifies patients more likely to benefit from extended therapy – particularly for late distant recurrence?
And ultimately: Can we get better at identifying who needs another 5 years of therapy – and who doesn’t?
