“ESMO 2026: What I am watching in breast cancer.
As ESMO begins in Madrid, I am following several clinical questions that could influence our daily practice.
- Survival: NATALEE’s six-year follow-up, postMONARCH, MA.40/FINER and the final OptiTROP-Breast01 overall survival analysis.
- Treatment after CDK4/6 inhibitors: FOURLIGHT-1 with selective CDK4 inhibition, updated VIKTORIA-1 findings, and evERA biomarker analyses.
- Immunotherapy: KEYNOTE-B49 in HR-positive/HER2-negative advanced breast cancer.
- Antibody-drug conjugates: PANKU-Breast01, neoadjuvant TALENT, and SWITCH, which addresses the increasingly common question of what to give after an ADC.
- Brain metastases: the DESTINY-Breast09 and TROPION-Breast02 CNS subgroup analyses.
- Individualising early treatment: extended endocrine therapy in NSABP B-42, treatment omission in TOP-1, and postoperative ctDNA surveillance.
My main questions will be: How large is the absolute benefit? Does it improve survival or quality of life? Which patients were included? And where does the treatment fit in the sequence?
These are presentations to watch. Conclusions about changing practice must follow the released results, their limitations and the full publications.”
