“TAILORswitch (aka PADA-2) is launched: great news for all switchies! (SIV campaign is ongoing).
In this academic trial, the French breast cancer research network Unicancer will further explore treatment switching in the 1st line setting for ER+ mBC, with a ctDNA test that is a true quantum leap for tumor fraction quantification (MRD WGS test).
Supported by a competitive grant from the Institut national du cancer and significant contributions from AstraZeneca and Natera, TAILORswitch will investigate two new approaches:
Drug treatment: the efficacy of an optimal switch from AI+palbo/ribo to camizestrant+abemaciclib in all patients with rising ctDNA levels (beyond ESR1m). This is a bit of a mash-up, SERENA-6 with an EMBER-3-like CDK4/6i switch. And -to reiterate-, the switch is triggered by the total fraction of ctDNA (all comers, like EMBER-3).
Drug treatment vs radiotherapy: a de-escalation of imaging in patients who are doing fine on AI+CDKi, as long as ctDNA is not rising swift*ly (here, trading-off new ctDNA surveillance vs less X-ray surveillance).
TAILORswitch is now being deployed in 32 sites across France (+ another country to join soon). Thanks to the many investigators and site staff + everyone involved at Unicancer / Institut national du cancer / AstraZeneca / Natera and my team + stat team at Institut Curie.
