“A major breakthrough for personalized cancer vaccines! ESMO – European Society for Medical Oncology
How does intismeran (mRNA-4157/V940) work?
An individualized mRNA cancer vaccine designed using each patient’s tumor mutations to generate neoantigens, activate and expand tumor-specific T cells, and enhance immune recognition and destruction of cancer cells.
Phase III INTerpath-001 evaluated intismeran + pembrolizumab vs pembrolizumab alone in 1,137 patients with resected high-risk melanoma.

Key Results (median follow-up 24.6 months):
- RFS: 82.2% vs 70.0% at 24 months
HR 0.58 (95% CI 0.45–0.75), P<0.001
42% reduction in recurrence or death - DMFS: 88.5% vs 80.2% at 24 months
- HR 0.62 (95% CI 0.45–0.85), P=0.002 38% reduction in distant metastasis or death
- Consistent benefit across most subgroups.
- Safety: Mostly low-grade, transient injection-related AEs, without apparent potentiation of pembrolizumab toxicity.
- OS remains immature.
An exciting step forward in precision immunotherapy!
However,
- manufacturing complexity
- turnaround time
- cost
- scalability
- equitable access
remain important challenges before widespread clinical implementation.”
