Aleix Prat: Ribociclib Plus Endocrine Therapy in High-Risk HR+/HER2-Negative Breast Cancer

“The RIBOLARIS journey continues.

Very happy to see the rationale and trial design of RIBOLARIS now published in Therapeutic Advances in Medical Oncology (Sage Journals).

RIBOLARIS was built around a simple but ambitious question: can we use the biological response of the tumor to neoadjuvant ribociclib + endocrine therapy to identify patients with high-risk HR+/HER2- early breast cancer who may safely avoid chemotherapy?

The trial brings together neoadjuvant therapy, molecular profiling and post-neoadjuvant treatment decisions, using the tumor’s response in vivo to help personalize what comes next.

Importantly, this could also translate into a remarkably simple precision-oncology strategy: for approximately 50% of patients, only 1-2 genomic assessments may be needed to guide treatment de-escalation, without the need for MRD testing or longitudinal molecular monitoring.

This article describes the rationale behind that strategy and the design of a study that has enrolled 1,100 patients.
