“TALAPRO-3 / 52% less progression – but at what cost?
NEJM 2026 / The Upfront vs Sequential PARP Debate
- 52% reduction in progression/death risk with talazoparib + enzalutamide + ADT (HR 0.48).
- BRCA-altered subgroup: Particularly impressive rPFS benefit (HR 0.37).
- Overall survival: Immature; no established OS benefit yet (HR 0.77).
- Toxicity matters: Grade ≥3 anemia 51%, RBC transfusions 40%, permanent talazoparib discontinuation 19%.
- The sequencing controversy: Only 10% of control patients subsequently received PARP inhibition.
- The unanswered question: Is upfront PARP inhibition truly superior to optimal sequencing? TALAPRO-3 did not directly compare these strategies.
