Ilyas Sahin: Giredestrant Studied in ER+/HER2− Advanced Breast Cancer

“New in NEJM:  The phase 3 evERA trial tested giredestrant, an investigational oral estrogen receptor degrader, plus everolimus in 373 patients with ER+/HER2- advanced breast cancer previously treated with CDK4/6 inhibitors.

Compared with standard hormone therapy + everolimus, median progression-free survival (PFS) was:

  • All patients: 8.8 vs 5.5 months (44% lower risk of progression or death)
  • ESR1-mutated tumors: 10.0 vs 5.5 months (62% lower risk)

No unexpected safety concerns emerged. What makes this interesting? The benefit was particularly strong in cancers with ESR1 mutations, which can make tumors resistant to hormone therapy and can be detected through a blood test. Overall survival data are still early, and the benefit in patients without ESR1 mutations is less clear. An encouraging step, especially for patients whose tumors carry ESR1 mutations.”
