NCI invites R01 applications to develop and test multi-level interventions that improve the transition from pediatric to adult care for childhood and adolescent cancer survivors. Projects should enhance risk-based surveillance, care coordination, and long-term engagement, address health disparities, and emphasize scalability and sustainability in real-world settings (STAR Reauthorization Act–aligned). Clinical trial optional.
Eligibility Criteria

Eligible: higher-education institutions; nonprofits; for-profits (including small businesses); government and tribal entities; independent school districts; public/tribal housing authorities; faith-/community-based and regional organizations.

Not eligible to apply: non-U.S. (foreign) organizations and non-U.S. components of U.S. organizations.
