Sachin Jain: SCAN’s Leadership in PACE Model Expansion via MyPlace Health
Sachin Jain, President and CEO of SCAN Group and Health Plan and an Adjunct Professor of Medicine at Stanford University School of Medicine, shared a post on LinkedIn:
“SCAN—inspired by Jennie Chin Hansen and Cheryl Phillips, two PACE pioneers who sit on its board—incubated and funded myPlace Health to serve some of the country’s most frail and vulnerable patients.
The PACE model of care remains the gold standard for nursing home-eligible patients who prefer to remain independent in their homes.
Mehmet Oz, the current administrator of Centers for Medicare & Medicaid Services, has notably been touting the PACE model as one of the best clinical models that the agency supports.
After incubating MyPlace, SCAN partnered with Commonwealth Care Alliance to fund and grow the business to its current footprint: 1 site in Downtown Los Angeles; and another site to launch this summer in Compton.
With Commonwealth Care Alliance’s sale to CareSource pending, SCAN acquired Commonwealth Care Alliance’s stake in myPlace Health to become the entity’s sole owner.

Sachin Jain detailed SCAN’s ongoing commitment to the PACE model, including its role in founding and expanding MyPlace Health. With full ownership now secured, SCAN plans to continue growing its footprint to support aging populations through this integrated care model.
