Vincent Rajkumar: To be a good clinician focus on getting a good history
Vincent Rajkumar, Professor of Medicine at the Mayo Clinic in Rochester, Minnesota, posted on X:
To be a good clinician focus on getting a good history. Especially when things are complex and not clear.
Good history taking is a special skill. It takes time and sound background clinical knowledge. The history is often far more important than all the tests in the world. No AI or tech can help if the history is not correct.
Many times I have to take the history on my own to be sure I have the story correct. When I’m not convinced or something doesn’t add up I take the history myself directly from the patient or their family. I have been misled by hearing the history from someone else (who may have heard it from someone else).
There is no substitute to getting the medical history correct in the first place.
