“Can a CD20×CD3 bispecific antibody improve first-line therapy for high-risk DLBCL?
Early results from the Phase III OLYMPIA-3 trial suggest that combining odronextamab with R-CHOP may offer a promising new approach for patients with previously untreated, high-risk diffuse large B-cell lymphoma (DLBCL).
Key findings from the Phase III safety run-in (Part 1B):
- Overall response rate (ORR): 90-95%
- Complete response (CR): 85% across both dosing regimens
- 75% of patients had high-risk disease (IPI 3-5)
Safety remained manageable:
- Neutropenia: 57.5%
- Cytokine release syndrome (CRS): 55%, predominantly Grade 1
- Immune effector cell-associated neurotoxicity syndrome (ICANS): 3 patients, all Grade 1 and fully resolved
- Addition of odronextamab did not compromise CHOP dose delivery
Based on these findings, the less frequent odronextamab dosing schedule was selected for the randomized Phase III portion of the trial.
While follow-up remains short and these data are preliminary, OLYMPIA-3 represents another important step toward incorporating bispecific antibodies into front-line treatment for high-risk DLBCL-an area where approximately one in four patients still fail to achieve durable remission with standard chemoimmunotherapy.”
