Yan Leyfman: Real-World Outcomes for Patients With Large B-Cell Lymphoma

CAR-T therapy has transformed outcomes for relapsed/refractory large B-cell lymphoma (LBCL), yet real-world data suggest that many eligible patients never receive it.

In an analysis of more than 10,000 patients with LBCL from the Flatiron Health Research Database, investigators examined treatment patterns and outcomes in the modern CAR-T era.

Key findings:

  • 13% of patients died within 1 year of starting first-line therapy without ever receiving second-line treatment .
  • Only 30% of patients proceeded to second-line therapy.
  • Even among patients considered eligible and fit for CAR-T, uptake remained limited.
  • Just 25% received CAR-T in the second-line setting.
  • Only 36% received CAR-T in the third-line setting.

Perhaps most striking, a substantial proportion of patients died before reaching the point where CAR-T could even be considered, highlighting the importance of identifying candidates early in the disease course.

These findings underscore a persistent gap between clinical trial success and real-world delivery of potentially curative therapies. Earlier referral to cellular therapy centers, stronger collaboration between community and academic practices, and improved patient education may help ensure more patients reach CAR-T before disease progression or clinical decline precludes treatment.

The challenge may no longer be whether CAR-T works – but whether eligible patients can access it in time.”
