“For decades, one of the greatest challenges in solid tumor immunotherapy has been finding safe and effective ways to target intracellular cancer proteins-targets that traditional antibodies simply cannot reach.
IMA401 offers a glimpse of a potential solution.
This first-in-human study evaluated a novel TCR-based T-cell engaging receptor (TCER) targeting MAGE-A4/A8 peptides presented by HLA-A*02:01, effectively allowing T cells to recognize tumor antigens hidden within cancer cells.
The concept is particularly exciting because cancer-testis antigens such as MAGE-A4 are expressed across multiple solid tumors while remaining largely absent from normal tissues, making them attractive immunotherapy targets.
Among heavily pretreated patients with advanced solid tumors, IMA401 demonstrated a manageable safety profile, with predominantly low-grade cytokine release syndrome and reversible cytopenias. Importantly, no grade ≥3 CRS was observed.
The efficacy signals are equally encouraging. Across patients treated at the recommended phase 2 dose, objective responses were seen in 20% of patients spanning 15 different tumor types. In head and neck cancer-the largest subgroup-response rates reached 29%, with a median duration of response approaching 9 months.
While still early, these results highlight the growing potential of next-generation TCR-based bispecific therapies to expand the reach of immunotherapy beyond traditional surface antigens and into the vast universe of intracellular tumor targets.
As the field continues to evolve, platforms like IMA401 may help unlock a new chapter in solid tumor immunotherapy-one where T cells can be redirected against cancers previously considered out of reach.”
