”Antibody-drug conjugates were built on a simple premise:
Use a monoclonal antibody to deliver high concentrations of a cytotoxic payload directly to cancer cells.
The premise held and delivered some interesting therapies to patients in the first wave. We faced then the period of learning, as we all know, there’s no failure in R and D there’s only learning. During this period, ADC technologies matured and expanded significantly.
We are now in the second phase, potentially merging into a 3rd phase and a wave a approvals accross several cancers are delivering clinical benefits to cancer patients.
In this Nature Medicine review, Mosele et al. argue that the pace of ADC innovation now exceeds clinical trial capacity. We have learned a great deal about ADC mechanisms of action and resistance in patients, and that understanding has produced a wealth of advances in construct chemistry and in rational therapeutic combinations.
The idea is focus on Integrating multiple chemistry advances into individual ADCs rather than testing them one at a time. Build the frameworks, infrastructures and tools needed to accelerate and de-risk preclinical and early clinical development. This review names the bottlenecks that anyone developing ADCs recognizes.
