Integrated Care for Older People (ICOPE) provides the framework to deliver person-centred, coordinated care. At the same time, Age-Friendly Cities and Communities create the conditions that make this care effective in people’s daily lives-through supportive physical and social environments, community action, and local systems. Whether you are a clinician delivering on ICOPE or an policymaker making your city more age-friendly, this workshop is for you. Bringing these two together is essential to maximise impact.

Age‑Friendly Cities and Communities (AFCC) provide a critical platform for making Integrated Care for Older People (ICOPE) real-where people live, age, and connect. While ICOPE strengthens person‑centred, integrated health services, AFCC ensures these efforts are embedded in supportive communities. Importantly, AFCC domains include health services and community support, enabling continuity of care across settings and sectors, and supporting older people’s functional ability.
