Can palliative care support delivered by social workers reach older cancer patients transitioning to skilled nursing facilities?
Older adults with cancer discharged from hospital to a skilled nursing facility face high six-month mortality and limited access to palliative care, often described as being ‘rehabbed to death.’ This protocol outlines a pilot randomised controlled trial of ALIGN, an intervention using virtual visits from palliative care social workers to support serious illness communication and care transitions.
Sixty patients and up to sixty caregivers will be randomised to ALIGN or enhanced usual care, with the pilot testing feasibility and fidelity ahead of a fully powered efficacy trial.
