Shernan Holtan: A Day in the Life of an Inpatient TCT Attending

Shernan Holtan, Chief of Blood and Marrow Transplantation and Professor of Medicine at Roswell Park Comprehensive Cancer Center, shared a post on LinkedIn:

“A day in the life of an inpatient TCT attending. A very normal, average, good day. Nineteen inpatients with approx. 400 decisions made, spanning several topics below.

Decision fatigue can be real. I think the sweet spot is 7 consecutive days inpatient. I used to do 14 days (many of us still might do this) on call, often followed by a full week working in the outpatient space or on research, which would translate to 19 consecutive work days. This is not at all unusual in our field.

Let’s be honest. The needs a break after awhile or performance falls. If we treat working our brains like a training a muscle, we recognize it needs to rest a bit between sets/reps to be able to perform well again.

We should create clinical schedules that optimize for performance, so we can bring our best the bedside every day.”
