ctDNA monitoring with Guardant360 every 2–3 months is costly and difficult to scale globally. We urgently need smarter strategies—when, how often, and for whom to test.

  • Highest ESR1m detection occurs between 6–30 months on AI + CDK4/6i (PADA-1)
  • SERENA-6 shows median ESR1m detection at 22 months, after ≥6 months on therapy.

Testing more frequently in this interval—and less after 30 months—may be reasonable.”
