“Everyone’s talking about what GLP-1s can do for cancer survivors, especially for the 19 types of cancer related to obesity. Almost nobody’s talking about what they can quietly cost.

Last week we hosted a GLP-1 and cancer event for the American College of Lifestyle Medicine’s Cancer Member Interest Group three takeaways that we discussed that I don’t think we’re talking about nearly enough.

First, to be clear: these medications are powerful tools in the toolbox and unfortunately too often there seems to be two camps: lifestyle or medications. This isn’t a knock on the drug. It’s a reminder of what comprehensive care should look like.

  • The muscle question (with a new twist). Early data shows aromatase inhibitors may blunt how well GLP-1s work. So what’s the reflex? Push the dose higher. But if we chase the number with more medication, are we doing it at the cost of skeletal muscle? For a survivor, lean mass isn’t optional. It’s tied to treatment tolerance, function, and survival. ‘Lose more weight’ cannot quietly become ‘lose more muscle.’
  • Mood (and sexual well-being and interest ). We track weight. We track metabolic markers. But are we asking about mood in a way that goes beyond a general screening? Low mood and depression deserve active screening on these meds, not a footnote. If someone’s motivation and joy start slipping, that’s not a side note. That’s not a compromise for weight loss.
  • Bone (the long game we keep deferring). Bone density moves slowly, so it’s easy to push down the list. But rapid weight loss affects bone, and ‘we’ll deal with it later’ is not a strategy for someone playing the long survivorship game

Here’s the through line: GLP-1s do the metabolic work. Protecting muscle, mood, and bone? That’s on us. Resistance training, protein, screening, connection. Not either/or. Both.

The medication is the catalyst. The care around it is what makes it last.
