“Most health systems run on single digit profit margins. Some are thinner than that.

Meanwhile the AI industry has spent the year bragging about how many tokens it can afford to burn.

Those two facts do not fit in the same room, and I think it is the most under discussed problem in clinical AI right now.

Healthcare does not need a model that can crack the one in a million diagnosis. That is television. What it needs is something cheap enough to run repetitive administrative work all day, on its own hardware.

That is a very different product than the one being sold into hospitals.”
