“Ambient AI scribes are now everywhere in medicine, and most of the physicians using them couldn’t tell you where their patient data goes.
The adoption curve is real. In one study, burnout in ambulatory clinics dropped from 51.9% to 38.8% after just 30 days on an ambient scribe. People are getting two hours a day back. So the tools work, and clinicians are right to reach for them.
But “it works” and “I understand it” are not the same thing. Most of us were never told the parts that actually matter.
Where does the audio of your patient’s visit live after the encounter? Who can access the transcript? Is it used to train the next model? What is the difference between a scribe that processes on the device versus one that ships everything to a server you have never heard of? And quietly the biggest one, are you even using it well, or just letting it draft notes you barely edit?
Most physicians are flying blind on all of it. That is not a knock on doctors. Nobody handed us the manual.
