“Less May Be More in Elderly Breast Cancer!
New Swedish cohort study suggests post-lumpectomy RT can be safely omitted in select low-risk, ER positive, T1N0 older women on endocrine therapy.
- Study: Swedish Breast Cancer Group, n = 601 (median age 71)
- Treatment: Lumpectomy plus 5 years endocrine therapy
- No radiotherapy
Results (median FU 119 months):
- Local recurrence 1.5% at 5 years; 5.5% at 10 years
- Contralateral 4.5% at 10 years
- OS 83% at 10 years
- Only 3 breast-cancer deaths
Key takeaway:
- Clinicopathologic features (small, luminal-like, non-aggressive) may suffice – no gene-assay needed.
- Omission valid only if patient commits to endocrine therapy for 5 years.
