Rami El Shafie: Secondary analysis of the CYBER-SPACE randomized phase II trial

“Happy to share the results of our secondary analysis of the CYBER-SPACE randomized phase II trial, published in Radiotherapy and Oncology by Annabella Schiele — and many thanks to the Heidelberg team for making this possible.

Objective:

To reassess established prognostic scores in patients receiving repeated SRS in combination with modern systemic therapies — a setting in which many traditional scores show limited validity.

Methods:

We analysed multiple prognostic tools (RPA, M-RPA, GPA, dsGPA, lung-mol GPA, BSBM, SIR, BMV) for their ability to predict:
Overall survival (OS)
WBRT indication (WBRTi) — defined as >10 brain metastases or leptomeningeal disease.
BMV was calculated at up to four distant brain failures.

Key Findings:

  • 202 patients; median OS: 13.1 months
  • BMV was the only score that reliably predicted both OS and WBRTi across all tumour entities
  • Other established scores showed limited prognostic value in the context of modern SRS + systemic therapy, especially for WBRTi
  • BMV remained prognostic through the 4th recurrence
  • Two BMV cut-offs clearly separated prognostic groups for OS and WBRTi

Conclusion:

Brain Metastasis Velocity (BMV) is the only consistent and reliable prognostic marker for both survival and WBRT indication in patients undergoing repeated SRS under contemporary treatment standards.”
