“MonumenTAL-3 is practice changing: talquetamab combinations beat DPd in relapsed myeloma with impressive PFS and OS benefit.
But the practical question is not ‘does talq work?’ It clearly does.
The question is: given dysgusia, weight loss, would I use it before teclistamab, elranatamab, or linvoseltamab for most patients?
I’m not sure.
Where I would strongly consider talq before BCMA bisoecific:
- As a bridge to BCMA CAR-T.
- In combination with tec for patients with EMD/high-risk biology.
I anticipate: BCMA bispecifics move frontline, and talq becomes a key 2L/post-BCMA or combination strategy.”
