Pierre Heudel: Hyperprogression after Immune Checkpoint Inhibitors

“Hyperprogression under immunotherapy: the essential contribution of real-world data in oncology.

Immune checkpoint inhibitors have transformed the management of many solid cancers. But in some patients, they can be accompanied by a rapid and unexpected acceleration of the disease: hyperprogressive disease.

The French multicenter study HYPERPROG, published in Cancer Communications, illustrates the value of Real-World Evidence to better understand these rare but critical situations.

From more than 13,000 patients treated with immunotherapy in 4 French centers, the authors identified 57 confirmed cases of hyperprogression.

The results show a very early evolution:

  • median time to onset: 31 days;
  • nearly 77% of cases in the first two months;
  • Median overall survival: about 4 months.

These results are a reminder that RWDs are not only used to complement clinical trials: they also make it possible to detect real, sometimes brutal, trajectories encountered in daily practice.

In oncology, better use of real-life data means better identifying warning signs, better monitoring patients at risk and adapting treatment decisions more quickly.”
