Some of the best responses are also the deadliest, we should not surrender randomization as it’s the only thing that protects us from doing harm.”

And for the most effective drugs, randomizing pts to the control arm becomes unethical. RCT are best for drugs with marginal benefit and encourage mediocrity in drug development.“
