The Reality Gap in Global Oncology

In limited-resource or conflict-affected settings, we often rely on data from landmark trials conducted in HICs. However, these trials rarely account for the infrastructure pressures that define our daily clinical reality.

International guidelines assume a “perfect state” of stability. They often overlook the realities of supply chain fragility, workforce strain and infrastructure volatility. In our environment, a single delayed shipment or power instability can halt treatment for weeks.

Sharing local data is vital for creating guidelines tailored to our specific patient populations.

Our work, “Cancer treatment amid armed conflict: Challenges and urgent needs in Sudan,” serves as a roadmap for this. By documenting how conflict reshapes care – from supply disruptions to the need for financial and logistical support – we provide the evidence necessary to build more resilient oncology systems.”
