Maximilian Merz: CAR T First Strategy Improves Outcomes Over Bispecific-First Approach

Maximilian Merz, Associate Attending Physician at Memorial Sloan Kettering Cancer Center, shared a post on LinkedIn:

CAR T or bispecifics

I’m convinced we’ve been asking the wrong question.

Very proud to share our new European 🇪🇺 real-world study published in Blood Cancer Discovery from AACR Journals

  •  640 patients, including a large cohort treated with bispecifics alone because they had no access to CAR T
  •  Not CAR T OR bispecifics
  •  CAR T first, bispecific at relapse, outperformed the reverse

CAR T AND bispecifics – and the order matters

For patients without CAR T access, bispecifics remain vital and highly effective. But where access allows, leading with CAR T looks like the best of both worlds.

Grateful to my fellow first and senior authors Tomas Jelinek, Nico Gagelmann and Carlos Fernandez de Larrea as well as all the collaborators and centers across Europe who made this possible:

Thomas Pabst, Raphael Teipel, Anca-Maria Albici, Bastian von Tresckow, Marcel Teichert, Uta M. Demel, Antonia Busse, Marie C Wesener, Tobias Holderried, Dr. med. Friederike Schmitz, Friedrich-Linus Rößiger, Fabian Mueller, Michele Hoffmann, Friedrich Stölzel, Natalia Tovar, Ben-Niklas Baermann, Martin Stork, Alexandra Jungova, Jiří Minařík, Jakub Radocha, Ivan Spicka, Ludek Pour, Polona Novak, Klara Šlajpah, Karla Rener, Ulrich Keller, Dr. Stephan Bohl, MD, David Fandrei, Patrick Born, Vladan Vucinic, Nicolaus Kröger, Irene Strassl, Natalie Schub, Roman Hajek, Matjaz Sever

The figure from the paper shows the state occupation probabilities by initial treatment. Patients with a CAR T first approach stayed longer in remission compared to a bispecifics-first approach, not only in first but also second line of TCE therapy after relapse.”
