Mashukur Rahman: Why Is 6 MV Still Used Everywhere in Radiation Oncology?

Mashukur Rahman, Resident Doctor at Bangladesh Medical University, Medical Officer at Directorate General of Health Services, shared a post on LinkedIn:

“If Higher MV = More Penetration
Why Is 6 MV Still Used Everywhere?

Because deeper is not always better.
Safer + More Precise = Better Treatment.

Why 6 MV is the Daily Workhorse?

  • Treats most routine cancers
  • Very predictable dose planning
  • Best suited for IMRT / VMAT
  • Minimal neutron risk
  • Standard shielding → affordable setup

When We Use Higher MV (>10 MV)-

  • Very deep tumors
  • Large body separation
  • Selected special cases

Trade-offs:

  • Higher neutron contamination
  • Higher bunker cost
  • Sometimes unnecessary extra dose spread

So,
Right beam = Right patient
Not highest energy = Best treatment
