“Are we ready to define the optimal salvage treatment after radiation failure for prostate cancer?
We are pleased to share our recent editorial letter published on European Urology Platinum in response to the growing discussion surrounding salvage treatment options for localized prostate cancer recurrence after radiation therapy.
The current evidence does not yet support a single salvage therapy as the standard of care for all patients.
While focal therapy has generated significant interest because of its potential functional advantages, the available literature still has important limitations. Current studies are largely retrospective, often involve heterogeneous patient populations and treatment modalities, and lack direct comparisons with contemporary salvage surgical series.
Importantly, there remains a significant evidence gap regarding outcomes of modern robotic salvage radical prostatectomy performed in carefully selected patients according to current European Association of Urology (EAU) criteria versus contemporary focal therapy approaches.
At present, major guidelines continue to emphasize individualized patient selection and shared decision-making, balancing oncologic control, functional outcomes, patient preferences, and treatment-related morbidity.
The field would greatly benefit from well-designed comparative studies evaluating
- Oncologic outcomes.
- Functional recovery.
- Quality of life.
- Retreatment rates.
- Long-term disease control.
Until stronger evidence becomes available, treatment decisions should remain patient-centered and guided by multidisciplinary discussion.
We look forward to future studies that will help define the optimal salvage strategy for patients facing radiorecurrent prostate cancer.”
