“Very pleased to share our new review in Therapeutic Advances in Medical Oncology, led by our unstoppable senior fellow Federico Monaca.

Cytokine release syndrome (CRS) is becoming increasingly relevant in solid tumours as T-cell engagers and other T-cell-redirecting therapies move from early development into routine clinical practice.

In this review, we discuss the mechanisms driving CRS, how the solid tumour setting differs from haematological malignancies, and the rationale for mechanism-based prevention and management strategies, including IL-6/IL-6R blockade, corticosteroids, IL-1 blockade, JAK/STAT inhibition and other targeted approaches.

For me, the key message is that we need more solid tumour-specific data. Current algorithms are still largely extrapolated from CAR-T and haematological settings, while the biology, disease burden, dosing schedules and treatment context can be very different.

Future priorities should include biomarker-driven risk stratification, harmonised CRS grading, prospective registries and rational trials of prophylactic or early intervention strategies, so that we can deliver these promising therapies more safely without compromising anti-tumour activity.”
