HDR Brachytherapy Boost after EBRT – New Insights from the EMBRACE II Trial
The recently presented EMBRACE II results at ESTRO 2025 provide compelling evidence that modern image-guided adaptive brachytherapy remains a cornerstone in the treatment of locally advanced cervical cancer.
1,482 patients from 49 international centers
- 3-year local control: 93%
- 3-year pelvic control: 94%
- 5-year overall survival: 82%
- Disease-specific survival: 89%
By combining IGRT-IMRT, concurrent cisplatin, and MR-guided adaptive brachytherapy (MR-IGABT), EMBRACE II achieved outstanding disease control while significantly reducing severe late toxicities compared with EMBRACE I.
Key message:
Advanced external beam radiotherapy alone is not enough. The integration of MRI-guided adaptive brachytherapy is essential for achieving optimal tumor control and improving survival outcomes in cervical cancer.
EMBRACE II establishes a new benchmark and reinforces MR-guided adaptive brachytherapy as the standard of care for locally advanced cervical cancer, particularly in high-risk patients.”
