Finally, some data – even though retrospective and just 39 patients – on one of the hottest unanswered questions in gynecologic oncology: can we safely and effectively rechallenge immune checkpoint inhibitors?
The RICE study, the largest multicentre retrospective series to date evaluating IO rechallenge in recurrent/metastatic cervical and endometrial cancers, provides encouraging real-world evidence.

Among patients rechallenged after disease progression:
- Objective response rate of 52% (vs 76% during the initial ICI exposure)
- Median PFS of 6 months, demonstrating clinically meaningful activity even after prior immunotherapy
- Manageable safety profile, with only 13% discontinuing rechallenge because of immune-related toxicity, substantially lower than during the initial ICI course
